English Section
Let us be honest from the start: health insurance is the biggest monthly bill most self-employed people pay, and it is the number one complaint among freelancers. An employee pays part of the premium and the company pays the rest, sometimes most of it, without the employee even noticing. You pay the entire premium out of your own pocket, every month, and you see the full number staring at you. That hurts, and it is natural to look for every legal way to soften the blow.
The good news: the law gives you a tax break of your own, one ordinary employees do not get. This chapter explains that deduction in detail, then covers the basics of buying through the health insurance Marketplace, the Health Savings Account, and ends with an honest note on how to think about this large cost.
The self-employed health insurance deduction
The basic rule: you can deduct 100% of the health insurance premiums you pay for yourself, your spouse, and your dependents, including children up to age 27. This deduction is available whether you itemize or take the standard deduction, because it is an “above the line” deduction, subtracted from your gross income before your taxable income is figured.
But there are conditions, and you must understand them well to avoid mistakes:
First: you must have net profit from self-employment. The deduction cannot exceed the net profit of the business you claim it under. If your net profit is $8,000 and your premiums are $12,000, you can deduct $8,000 only, and the rest does not carry forward to next year. The practical rule: this deduction is for those who make a profit, not those who take a loss.
Second: you cannot claim the deduction for any month in which you were eligible to participate in an employer-subsidized health plan, whether through your own employer or your spouse’s. If your spouse is an employee and you could join the company plan, you get no deduction for those months, even if you choose not to join. Watch this point carefully, because it is one of the most common sources of errors.
Third: the plan must be in the name of your business or in your name as a self-employed person. If you are paying premiums to continue coverage from a former job, those premiums qualify too, as long as you are paying them yourself now.
Where does it go on the return? On Schedule 1, among the adjustments to income. It reduces your income tax, but note this: it does NOT reduce your self-employment tax. Many people assume every deduction cuts every tax, and that is not true here. The deduction saves you income tax only, and self-employment tax is figured on net profit before this deduction.
A quick example: a freelancer with $60,000 of net profit pays $9,000 a year in health insurance premiums. She can deduct the full $9,000 because it is less than her net profit, so she pays income tax on $51,000 instead of $60,000. But her self-employment tax is figured on the full $60,000. The difference matters for understanding your real bill.
The health insurance Marketplace (ACA), simply
If you do not have coverage through a spouse or anywhere else, the natural place to shop is the health insurance Marketplace, known as Obamacare or the ACA, at healthcare.gov or your state’s exchange. The basic idea is simple: you enter your expected income, the site figures the assistance (premium tax credit) you qualify for, and your monthly premium drops right away.
The practical points you should know:
Open enrollment is usually at the end of the year for the following year, but starting self-employment counts as a “qualifying event” that opens a special enrollment period for you. Do not wait until November if you started your business in March.
The assistance is based on your income estimate. If you estimate too low, you pay back the difference when you file your tax return (through Form 8962). If you estimate too high, you get the difference back as a refund. So update your income estimate in your Marketplace account whenever your actual income changes, especially since you are self-employed and your income fluctuates. This is the most important practical tip in this section: do not set an estimate in January and forget it until December.
Plans are divided into metal tiers: Bronze has the cheapest premiums and the highest costs when you get care, Gold and Platinum cost more per month and less when you get care. Do not automatically pick the cheapest. Figure the total expected yearly cost: the monthly premium times 12, plus the plan’s out-of-pocket maximum. Someone who sees the doctor often may save money with a more expensive premium.
The Health Savings Account (HSA): the triple advantage
If you choose a High Deductible Health Plan, you become eligible to open a Health Savings Account, one of the strongest tax tools in the entire American tax code. Why? Because it carries a rare triple tax advantage: money you put in is deductible from your income, earnings inside grow tax-free, and withdrawals for qualified medical expenses are completely tax-free. Very few accounts give you the tax break at all three stages.
The contribution limits for 2025 were $4,300 for individual coverage and $8,550 for family coverage, verify the 2026 figures at filing because they change yearly. If you are 55 or older, you can add an extra $1,000. Note that these limits cover contributions from all sources combined, so do not exceed them.
Eligibility: you must be enrolled in a high-deductible plan that meets the IRS minimums set each year (check the current number when choosing your plan), you must not have other disqualifying health coverage, and nobody can claim you as a dependent on their return.
Why do the self-employed love it? Because the money does not vanish at year end the way it does in flexible spending accounts. Your HSA balance rolls over year after year, stays with you even if you change plans or jobs, and you can invest it so it grows like a retirement account. Many professionals advise paying medical bills out of pocket when you can, letting the HSA grow for retirement years, and keeping the receipts.
What counts as qualifying premiums?
Not all insurance payments are equal in the eyes of the law. Premiums eligible for the deduction include: basic medical insurance, dental and vision insurance, and Medicare premiums (including Part B and Part D) if you pay them yourself, plus continuation coverage premiums after leaving a previous job. Long-term care insurance counts within dollar limits that depend on your age and change yearly, check the limit in effect for your year before claiming the full amount.
And what does not count? Premiums paid with pre-tax dollars through an employer’s plan (yours or your spouse’s) cannot be deducted a second time, this deduction is for those who pay out of pocket after tax. And do not forget the monthly eligibility rule: any month you were eligible for an employer-subsidized plan, the deduction falls away for that month even if you did not enroll. If your spouse starts a new job with a health plan in July, your deduction stops in July, not at year end.
One last point: coverage for your children up to age 27 qualifies for the deduction even if they are no longer dependents on your tax return. Many self-employed people do not know this detail and miss a deduction they deserve.
An honest word: insurance is expensive, and here is how to think about it
We will not sugarcoat the truth: health insurance in America is expensive, and the self-employed person pays the whole price. You may find yourself paying $600 or $800 or more than a thousand dollars a month for a family, and that amount stings. But think of it this way: insurance is not a club membership, it is protection against catastrophe. A single night in the hospital without insurance can cost more than five years of premiums. You do not buy insurance because you expect to get sick, but because your entire business, and all your savings, can vanish with one medical bill.
The practical way to think: compute the “total yearly cost” of each plan you consider, meaning annual premiums plus the worst-case out-of-pocket scenario. Then ask yourself: does the network of doctors and hospitals include the ones I need? Are my medications covered? The cheapest premium is not always the cheapest in reality.
And remember finally: the self-employed health insurance deduction, the Marketplace assistance, and the HSA are all tools the law created to help you carry this cost. Use all of them if you can. Being insured and tax-organized is part of being a professional business owner, not a luxury.
